Bayshore Medical Personnel is excited to announce a full-time contract opportunity for an RN Case Manager to join our team in Saint-Jean-sur-Richelieu, QC! We're looking for dedicated professionals to make a meaningful impact in Military Health Services.

CMN’s work as a member of a multidisciplinary primary health care team in a primary care/clinic setting. Their primary responsibility is to complete detailed case management assessments, develop comprehensive care plans that reflect individual health care needs, and monitor clients and make suggestions for modifications as necessary.

Successful candidates will possess 3 full-time equivalent years of clinical experience within the last 4 years in nursing practice, including previous case management experience.

Candidates must also possess:

  • Baccalaureate Degree in Nursing from a recognized university
  • Certificate of Basic Life Support (BLS) for Healthcare Provider or CPR/AED Level C. Annual recertification is required 
  • Current license to practice from a provincial or territorial College of Nurses in the province of practice
