The Work You Will Do:
As a Complex Care Nurse Lead you will work to support, mentor and train your team to meet the diverse needs of the clients, caregivers and nurses you serve in your community
Assessment Delivery
Visit new and existing clients, caregivers and nurses on a regular basis. This role is an active community position that requires regular home visits within the community. This role involves a high level of hands-on care, training and coaching including weekly onsite visits with clients and caregivers and nurses in the field.
Leadership
· Supervise, support and mentor field caregivers and nurses
· Audit and provide regulatory compliance for provincial and in-house best practices
· Visit clients and staff for in-person supervisory visits
· Staff engagement, coaching and support as required
Training & Development
· Coordinate and provide training for field caregivers and nurses
Community Care
· Provide hands-on clinical/nursing care
· Build client relationships
· Provide direct client care on an as-needed basis
Administration
· Dedicated administrative requirements to be met on a weekly basis
· This position will require approximately 70% Administrative and 30% Client-Facing requirements
