CarePartners is one of Ontario’s largest, most reputable and well-established accredited home health care providers, providing nursing, personal support, home support, palliative, therapy and rehabilitation support services for patients of all ages across urban and rural Ontario. We visit approximately 80,000 patients each year in homes, schools, clinics, workplaces and retirement homes, spread across 14 branches and 21 nursing clinics.
We are currently looking for someone to join our dynamic Nursing Team in the role of Clinical Practice Lead. This person will work closely with our provincial Professional Practice Research & Education (PPRE) team and support clinical standards, orientation, and training. They will collaborate with leaders and help align strategic goals, priorities and initiatives set out and defined by our unique patient populations and Ontario Health Teams (OHTs).
We are looking to fill a Temporary 20-month Full-Time Clinical Practice Lead position in the Waterloo-Wellington - Waterloo Branch.
Participation in the Branch On-Call schedule is required, which includes evenings and weekends on a rotational basis.
In addition to providing home-based personal support, rehabilitation and therapy, nursing and palliative care across Ontario, CarePartners also serves the community through clinics, transitional care units, and provides relief in nursing homes and shared care settings. Since 2009, through our outreach program, we’re also proud to work in collaboration with international agencies to organize medical care and clinics in countries with poor access to health care.
