About the role:
 
As a Health Care Technician (HCT), you provide direct hands-on care to patients in the comfort of their homes under the guidance of a Directing Registered Nurse (DRN), whom you connect to virtually. This is an exciting opportunity for you to make a difference in the lives of patients of all ages, including those receiving end-of-life care, managing complex conditions, and pediatric patients. You will receive training to enhance your skills and expand your practice, enabling you to deliver exceptional care and support.

What you'll do: 
  • Under the direction of the DRN:
    • Observe and document the patient's electronic medical record
    • Provide personal care support and assist with activities of daily living and medication administration
    • Engage with the family/caregiver
    • Identify additional resources required in the patient's home
  • Communicate with the DRN through Smartphone Technology via a real-time electronic monitoring system
  • Collaborate with the DRN to develop and modify the patient's care plan based on patient outcomes
  • Perform other duties as assigned
What we are looking for:
  • Personal Support Worker (PSW) certification
  • Developmental Support Worker (DSW) certification will be considered
  • One to two years' experience as a PSW
  • Current CPR and First Aid certification
  • Fundamentals of Palliative Care- Level I (or a commitment to complete within one year)
  • Experience in Palliative or acute care (hospital) preferred
  • AHPCE course completion is an asset
  • Have a personal vehicle and possess both a current driver's license and proof of vehicle insurance
  • A current and original copy of a satisfactory Criminal Records Check
  • Must be able to wear Personal Protective Equipment (PPE)

Working conditions and physical demands: This role requires a detail-oriented approach in a dynamic environment, with physical activity including lifting, carrying (using proper techniques), bending, reaching, kneeling, and other movements that emphasize good body mechanics.
