Calian recherche actuellement un/une Infirmier(ère) gestionnaire de cas pour une opportunité de travail à la BFC St-Jean. Comme infirmière bachelière, spécialisée dans la gestion des cas, vous devrez fournir une assistance et un soutien aux Forces canadiennes (FC) et à leurs familles qui demandent diverses prestations et services de soins de santé du MDN et d’autres sources afin d’améliorer leur état de santé et de promouvoir leur bien-être. Les activités principales comprennent:

Responsabilités

  • Effectuer des évaluations de gestion des cas (GC) complets en utilisant le processus de gestion des cas. Fournir des services de GC centrés sur le client pour aider le retour du client au service actif ou à la vie civile;
  • Évaluer et superviser le progrès du patient jusqu’à son rétablissement et son retour au travail;
  • Consulter d’autres professionnels de la santé pour assurer qu’un plan de soins complet est développer ;
  • Coordonner et suivre les demandes de prestations avec divers ministères et organismes tels que ACC, la Financière SISIP, RPC/RRQ etc;
  • Faire des présentations afin de promouvoir le programme de GC à des unités, des organismes internes et externes;

Qualifications

  • Détenir un baccalauréat d’une université reconnue en sciences infirmières (obligatoire)
  • Membre en règle de l’Ordre des infirmières et infirmiers du Québec (OIIQ)
  • Minimum de trois (3) ans d’expérience à temps plein de pratique infirmière à temps plein au cours des 4 dernières années dans les domaines suivants :
  • Excellente maîtrise du français et de l’anglais requise pour exercer les fonctions du poste.

Informations supplémentaires

  • Contrat à temps plein, 37.5 heures par semaine (7.5 hrs/jour)
  • Heures d’ouverture de la clinique du lundi au vendredi, de 7h30-16h00
  • Poste contractuel de remplacement, dont la fin est actuellement prévue au 18 décembre 2026. Aucune prolongation n’est prévue à ce stade.

Through the Health Care Provider Contract, Calian employs health care professionals across Canada who are integral to supporting the health and wellbeing of our military members.

We are currently seeking a Registered Nurse (RN) Case Manager who would be interested in a full-time position at CFB St-Jean.

Responsabilities

The RN Case Manager is a key member of the primary health care delivery unit team and is responsible for providing case management services to CAF members who suffer from protracted and complex health issues. They facilitate access to health services based on the clients needs and programs and benefits for which they are eligible. They are directly involved in patient care coordination and conduct comprehensive case management assessments using the case management process with a goal of assisting patients return to active duty or to civilian life. The Case Manager works in collaboration with a multi-disciplinary team to ensure comprehensive care plans are developed for each patient.

Qualifications

  • Bachelor’s Degree in nursing (must-have)
  • Licensed and in good standing with l’Ordre des infirmières et infirmiers du Québec (OIIQ)
  • Current CPR certification (for healthcare provider or level C)
  • 3 years of full-time experience within the last 4 years performing assessments and developing care plans that reflect client’s individual needs as well as experience providing health education programs in a community health or primary care setting.
  • A high level of proficiency in both French and English is required for this role

Additional Information

  • Full-time contract, 37.5 hours per week (7.5 hours per day)
  • Clinic operating hours: Monday to Friday, 7:30 a.m. – 4:00 p.m.
  • Short-term contract with an anticipated end date of December 18, 2026. At this time, no extension is expected.
