HOMEBASE is part of a global movement in Integrated Youth Services (IYS), designed to connect youth and families with the right services, in the right place, at the right time. Our multidisciplinary, collaborative approach emphasizes early intervention and holistic care.

The Registered Nurse plays a vital role in promoting health and preventing illness and enhancing access to care.  This position supplements existing primary health care services by identifying health needs and risks, developing and implementing health programs, delivering direct patient care and collaborating with other healthcare professionals to support community well-being.

As a member of the Mobile Integrated Health Team, the nurse works closely with YMCA Homebase staff to serve diverse populations, including those requiring mental health, addiction, sexual and physical health services. The role involves close collaboration with a wide range of interdisciplinary partners, such as Community Mental Health and Addictions, Primary Health Care, Psychiatry, Paramedic Services, justice and victims' services, child and family services, and police.

Duties include but are not limited to:

  • Assess patient health care needs (physical, emotional, psychological, and spiritual) to identify appropriate nursing interventions;
  • Monitor, record and report symptoms and changes in patients’ conditions;
  • Determine the most appropriate treatment, healthcare provider, or medical equipment based on patient needs
  • Administer medications and treatments as prescribed by a physician/nurse practitioner;
  • Provide nursing care and treatment (including complementary therapies and/or counseling) for health problems;
  • Under medical directive administer vaccines/immunizations;
  • Perform point of care testing (i.e. blood glucose, rapid antigen detection test, etc.);
  • Administer/interpret results of mantoux tuberculin test;
  • Help patients to identify and use health resources, including those available in the community and from community agencies;
  • Initiate health education and other activities that assist, promote and support patients as they strive to achieve the highest possible level of health;
  • Create learning resources for nurses and other health care providers;
  • Develop and deliver health education programs for patients or communities.
  • Coordinate and expedite patient flow by triage;
  • Handle patient phone calls, provide telephone advice, and arrange for follow-up appointments as necessary;
  • Collect accurate and comprehensive patient-related information and document on clinical management system;
  • Monitor supply levels and/or expiration of medical and clinic supplies, and order supplies as needed from approved vendors.

Qualifications & Requirements:

  • Bachelor’s degree in nursing with a minimum of two (2) years clinical experience, preferably in community-based health care;
  • Active registration with the College of Registered Nurses of Saskatchewan (CRNS);
  • Current Basic Cardiac Life Support (BCLS) Certification (ACLS an asset);                       
  • Demonstrated ability to provide initial psychological interviews and assessments;
  • Proven experience working within interdisciplinary teams, contributing to program development and managing complex cases
  • Must have a valid Driver’s license;
  • Must have a clean criminal record and vulnerable sector check;                                  
  • Training in crisis intervention, suicide risk, or violence risk assessment is an asset.;                                                                                                                                                                                                                                                                                        
  • Proficiency in Microsoft Office Suite & Teams, Google Drive etc. is considered an asset;    
  • English Required; French not required but would be an asset;
  • Demonstrated ability to provide initial psychological interviews and assessments;
  • Requires independent critical thinking, clinical judgment and decision-making skills to develop, implement, and monitor care plans;
  • Always conduct business with the highest standards of personal, professional and ethical behaviour;
  • Demonstrated ability to influence, advise and build trust with various stakeholders.

Schedule:

This position includes three 4-hour shifts on the following days:

  • Tuesdays and Thursdays from 1500 to 1900 hours (3:00 PM – 7:00 PM)
  • One Saturday per month from 1200 to 1500 hours (12:00 PM – 3:00 PM)
