What The Role Involves

The below noted tasks are to be completed in a manner that builds the business and meets the strategic needs of the organization.

 

Strategic Change Leadership

  • Develop and implement comprehensive change management strategies to support organizational objectives, starting with the Digital Transformation agenda
  • Collaborate with senior leadership to align change initiatives with strategic goals

Change Management Planning

  • Design and execute change management plans, including risk management and communication strategies
  • Collaborate with project managers to ensure change management plans are integrated into project timelines and budgets
  • Follow Organizational Change Management (OCM) methodologies to conduct assessments of the current state, analyze partners, identify risks, and establish the goals and milestones of changes
  • Maintain working knowledge of existing business processes to understand the impact of changes

Relationship Management and Partner Engagement

  • Build strong relationships with internal and external partners
  • Develop and deliver key communications to partners, ensuring timely and effective information dissemination
  • Manage partner expectations and address concerns proactively and with exceptional skill/expertise

Resistance Management and Adoption

  • Identify and mitigate resistance to change through targeted strategies
  • Foster a culture that supports change adoption, and reinforces new behaviours
  • Ensure sustainability and long-term success of change initiatives using frameworks and actions that help to embed new processes, behaviours, and practices into the organization’s culture

Training and Development

  • Design and oversee the delivery of training programs and transition plans to equip employees with necessary skills for change adoption
  • Ensure training in change initiatives aligns with organizational needs and strategic objectives

Performance Monitoring and Evaluation

  • Define and track success metrics for change initiatives
  • Conduct regular assessments to ensure change management activities meet organizational goals
  • Administer post-implementation monitoring processes to measure the results of changes and
    make necessary course adjustments

Leadership and Coaching

  • Provide guidance to project teams on integrating change management into project plans
  • Build knowledge capacity though coaching of senior leaders and managers on effective change management practices

Collaboration and Integration

  • Work closely with various departments to ensure alignment and coordination of change efforts
  • Facilitate cross-functional collaboration to drive successful change implementation
  • Provide input to strategic decisions that affect the functional area of responsibility

What You Bring

 

  • Relevant post-secondary education and/or combination of education and experience
  • Change management certification (e.g. Prosci, APMG, CCMP)
  • Lean Six Sigma or PMP certification is considered an asset
  • Strong understanding of evidence-based change management principles, methodologies, and tools
  • Proven ability to lead strategic change initiatives and align them with organizational goals
  • Excellent communication and interpersonal skills, with the ability to build strong and effective relationships
    with diverse partners
  • Ability to navigate conflict and manage difficult conversations in particular around resistance to change
  • Strong analytical capabilities to assess change impact and develop effective mitigation strategies
  • Ability to navigate complex organizational dynamics and adapt to changing environments
  • Strong proficiency in all aspects of Microsoft Office, with proven skills in Word, Excel, and PowerPoint
  • Strong organization, problem-solving, and critical thinking skills
  • Knowledge of project management tools, techniques, and methodologies
  • Home and community care industry experience an asset
  • Proficiency in French is considered an asset
  • A clear background check
  • Valid driver's license, own vehicle, and appropriate insurance

In addition to providing home-based personal support, rehabilitation and therapy, nursing and palliative care across Ontario, CarePartners also serves the community through clinics, transitional care units, and provides relief in nursing homes and shared care settings.

Through our outreach program, we’ve been organizing medical care and clinics in countries with poor access to health care since 2009.
